It's hard to say whether some wild barbarian would take this wish literally. But a modern person will not rush at the interlocutor, having heard this, and will not sharply deprive himself of an eye in order to demonstrate a plucked eye.
That which is simply unbearable
"Gouge out your eye" - so they can talk about something very sour, if it knits, causes soreness. For example: Apples in the forest are useless - tiny in size, gouge out the taste of your eyes. You can also say: "So that it hurts."
