The record book accompanies students throughout the learning process. This is a real document of the student, which records the passage of the program of study.

Here are the basic rules for filling it out:

  1. Each record book must have a registration number. The HR department of the educational institution assigns an identification code to the student, which exactly matches the registration code.
  2. The first sheet of such a document is filled in with a black or blue pen. This page contains individual information: the name, patronymic and surname of the student, date of issue, faculty and speci alty, book number. By the way, the name, patronymic and surname should also be written on all sheets.
  3. Gradebooks must contain the rector's signature in the column specially designated for this purpose.
  4. The next person who must also sign the document is the head of the department.
  5. Under the photo of the owner must be his signature.
  6. Each of the above signatures requires the seal of an educational institution.
  7. The "Transferred" field is required if a student is enrolled from anothereducational institution. If a young person is just starting to study at a university, then the “Entered” field is filled in, indicating the year of admission and the course of enrollment.
  8. When an erroneous entry appears, it is necessary to make corrections with a pen, crossing out the incorrect information. After correcting, the new data must be certified by the signature of the head of the department for the organization of the learning process.
  9. As a rule, the student's record book is not handed out. The department for organizing the learning process is responsible for its safety. Upon request, a copy may be provided to the student within five business days.
  10. If this document is still kept by the student, then it would be useful to have a cover for the grade book.

Record keeping:

  1. The record book is filled in by one of the employees of the educational unit.
  2. After the end of the first and third semesters, information about the completion of training courses by the student is entered.
  3. The authenticity and reliability of information about passing the final test or exam is certified by the signature of the teacher or head of the department who accepted it.

    In the record book in accordance with the curricula - administrative and licensed - are entered:

  • marks on the results of passing exams and information about the courses of lectures listened to by the student in the "Theoretical course" section of the required semester;
  • marks on the results of passing tests and information about the corresponding course in the section"Practical studies" of the desired semester;
  • information about the internship and the results of the defense of the report on this process in the section "Internship";
  • marks on the results of the defense of term papers/projects in the section "Term papers/projects";
  • marks on the results of passing the last certification exams in the section "Final certification exams";
  • marks of the results of passing the final qualifying work/project in the speci alty/direction under study in the "Graduation paper/project" section.
